RN Household Team Leader Healthcare
Landis Homes is searching for an experienced RN to work in our Healthcare households!
Full-time: Monday through Friday
- 10:30 p.m. to 7 a.m., up to one weekend per quarter, 72 hours bi-weekly

Position Essential Duties/Responsibilities:
- Facilitates team members, residents, and family members in shared problem solving, decision-making, and planning.
- Plans, implements and evaluates nursing care for the residents.
- Administers medications and performs treatments in accordance with physician orders and nursing practice.
- Completes and documents assessments, ensuring that resident rights and Honoring Lives principles are incorporated into the resident's care plan.
- Assumes responsibility to ensure that documentation is completed as required for all disciplines.
- Responsible for resident care plans Interdisciplinary Care Plans and Resident Care Summary.
- Completes MDS Resident Care Checklist.
- Assists with direct care, such as bathing, assists in the dining room, responds to call bells and participates in resident activities as able.
- Monitors performance of inter-disciplinary team members in the Household and provides input to performance evaluations.
- Functions as a team member in the Household where assigned, working collaboratively to ensure integration of all staff.
- Provides direct care services as needed for the health and safety of residents.
- Models, teaches and utilizes effective communication skills such as active listening, giving meaningful feedback, communicating ideas clearly, addressing emotional behaviors, resolving conflict and understanding the role of diversity in communication.
- Reports significant events and unusual occurrences to the Nurse Manager or to the Campus Supervisor in a timely manner.
- Investigates events that occur on a particular shift to ensure that appropriate interventions are in place.
Qualifications:
- Experience necessary.
- RN license in the state of PA.
- CPR certification
